How to change my registered name?

Please submit the following information:

(1). Print out "Authorization of Personal Info Change /Account Closure" which needs to fill in and signature.

(2). The front and the back of your ARC /Taiwan Identity Card.

(3). Please upload a photo of yourself handholding ID card/ARC card and handwritten notes. The handwritten note with the writing of "Authorization of Personal Info Change", date and your signature. Please confirm the photos you uploaded are clear enough to identify

(4). To change your name: Please provide "Personal Household Certificate "

(5). Please email the information to [e-mail], and the email title is "Authorization of Personal Info Change" (If you are under the age of 18, please also attach the guardian's identity document to show your consent, we recommend using the household certificate.)

Notice: your register bank account would be invalid, you must change a new name through bank and contact with us.
